Nursing Care Plan for Herniated Nucleus Pulposus
Nursing Diagnosis : Impaired Physical Mobility related to pain and discomfort, muscle spasm such restrictive therapy : bed rest, traction, neurovascular damage.
Purpose : Do not damage the physical mobility.
Expected outcomes :
Clients express understanding of the situation / risk factors and individualized treatment rules.
Demonstrating behavioral techniques are possible.
Maintain or improve the strength and function of diseased body parts and or compensation.
Nursing Intervention - Independent :
Provide security measures as indicated by the specific situation.
Note the emotional response / behavior on immobilization . Provide activities in accordance with the patient.
Follow activities / procedures with methods break . Instruct the patient to remain. participate in the daily activities within the limitations of the individual.
Help the patient to perform active range of motion exercises or passive.
Instruct the patient to train the lower leg / knee. Value of the edema, erytema on the lower extremities.
Assist patients in progressive ambulation activities.
Demonstrate the use of auxiliary tools such as a walker, a cane.
Give with good skin care , massage point setelahsetiap depressed position changes. Check the condition of the skin under Brace, with a specific time period.
Collaboration :
Give pain relief medication approximately 30 minutes before moving / doing patient ambulation.
